• If you are not able to complete the New Patient Packet before you come, please allow at least 30 minutes to complete it in the office before your visit with the doctor.

If your computer will allow you, to either directly print, or download the New Patient Packet, print and complete it before your appointment. Bring the completed form with you when you come in. Be patient, the time it takes for the form to appear depends upon your internet connection speed.

Find out what the office visit co-pay is for your particular insurance company, and make sure that you have that amount with you when you come.

• Co-pays are collected the day of the appointment.

• Payment can be in the form of cash, personal check or credit card. We accept American Express Mastercard, Visa, and Discover.

Be sure to have your insurance card(s) and driver's license(s) with you when you come in.

•
We accept insurance payments from Blue Cross, United Healthcare, CIGNA, Tricare, and AETNA.
Medicare and Medicaid patients are also warmly welcome in our office.
