Post Operative Handouts, Amended Permission & Returning Patient Forms:
If you have recently had a surgery or have one scheduled, you should have received the appropriate pre-op or post-op handout from our office. But, if you need a replacement or additional copy of the instructions you received from our Surgery Scheduler, or are coming into the office for a return visit of any kind, and want to complete your paperwork before arriving, this is the page for you:
1.* You may select the appropriate Pre- or Post-Op Educational Handout, and either view it online or print it. Use the pop-up menu below, "Pre/Post-Op Handouts." 2.* You may download a variety of Alabama ENT Associates office forms and save them for printing later on a Macintosh or Windows computer at home. Use the "Download Forms" pop-up menu below. Be patient, the time it takes for the form to appear depends upon your internet connection speed.
These handouts and forms are also all available in our office if you have difficulty obtaining them from the website. Stop by anytime the office is open and ask. (Please do not call our office for technical assistance.)
